Requesting Family and Medical Leave FMLA
Employees at the University of Washington (UW) or the state of Washington who are professional, classified staff can request Family and Medical Leave Act (FMLA) if they have a serious medical condition or need to care for a family member.
Request Process
- Give at least 30 days’ notice if you know in advance of your need for a leave of absence. If you learn of your need for time off or a leave of absence less than 30 days ahead of time, request it as soon as you are able.
- The employee and their healthcare provider will fill out the appropriate FMLA form. Health-care provider certification forms:
- FMLA (campus) Parental Leave for Birth Parent (PDF)
- FMLA (campus) Parental Leave for Parent other than Birth Parent (PDF)
- FMLA (campus) Personal Serious Health Condition (PDF)
- FMLA (campus) Family Member's Serious Health Condition (PDF)
- FMLA (campus) Serious Injury or Illness of Military Family Member (PDF)
- FMLA (campus) Military Family Qualifying Exigency (PDF)
- The completed form is faxed or mailed, by the employee or their doctor, to Campus Human Resources for review.
- Once the employee’s eligibility for FMLA has been approved, the employee will receive an email and Workday will be updated.
